BENEFICIARY:

· 2 valid ID
· Marriage Contract (if spouse)
· Latest Proof of Remittances
· Latest Electric or Water Bill (Must be original copy/not necessarily under borrower’s name)
· 2 Proof of Residency (Documents with the name of the borrower and same address in Electric or Water Bill.)
CO-BORROWER : Immediate Relative WITH INCOME
· 2pcs. 2x2 picture
· 2 valid ID
· Proof of Income – Payslip, Company ID, COE (if working); Business Permit (if owns a business).
· Latest Electric or Water Bill (Must be original copy/not necessarily under borrower’s name)
· 2 Proof of Residency (Documents with the name of the borrower and same address in Electric or Water Bill.)
EXAMPLES OF PROOF OF RESIDENCY:
